This sweepstakes (“Promotion”) is sponsored by Notable (“Sponsor”), located at 400 S. El Camino Real, STE 600, San Mateo, CA 94402. Any references to “we,” “us,” or “our” refer to the Sponsor.

No purchase or payment is necessary to enter or win. A purchase does not increase your chances of winning.

Who Can Enter: Open to legal residents of the 50 United States and the District of Columbia who are at least 18 years old (or the age of majority in their state of residence) at the time of entry.

Who Cannot Enter: Employees, officers, and directors of Sponsor, its affiliates, subsidiaries, advertising and promotion agencies, and the immediate family members (spouse, parents, siblings, and children) or household members of each are not eligible to participate.

The Promotion begins on Monday, February 17, 2025, at 10:00 AM CT and ends on Friday, February 21, 2025, at 5:00 PM CT, corresponding to the HIMSS 2025 conference. Any entries received after the end date/time will not be eligible.

Token Scan & Pass Method: During the conference, you may receive a physical token (e.g., a card) representing a specific nonprofit organization. Each token has a QR code:

• Scan the QR code with your mobile device to access the entry form.
• Provide the requested information (e.g., name, email address) and submit.
• Pass the token to another individual at the conference, who can then scan and enter.
• You will be part of that token’s “flow” (chain of scans).

Alternate Method of Entry (AMOE): If you do not receive a token, do not wish to scan, or cannot attend the conference in person, you may still enter by visiting one of the following sites/flows.

Mailed entries must be postmarked by February 19, 2025 and received by February 21, 2025 to be eligible.

Charitable Donation: The nonprofit organization whose token generates the “longest flow” (i.e., the highest number of valid, unique scans during the Promotion Period) will receive a $10,000 donation from Sponsor. This donation is not part of the sweepstakes prize for individual participants.

Sweepstakes Prize: Each participant who scanned (or otherwise entered via AMOE) for the winning token’s flow will be entered into a random drawing for a chance to win $1,500 (the “Prize”). Sponsor reserves the right to adjust the prize amount; any change will be noted in these Official Rules.

Winning Nonprofit (“Longest Flow”): After the Promotion ends, Sponsor will determine which token flow received the most valid, unique scans.

‍Random Drawing: All eligible participants in that winning token’s flow (including any AMOE entries that identified that token) will be entered into a random drawing for the $1,500 Prize.‍

Odds: The odds of winning depend on the number of eligible entries in the winning token’s flow.

The potential winner will be notified via the email address provided at entry (or on the AMOE card) within 7 days after the conclusion of the Promotion.

If the potential winner does not respond and fulfill all Promotion requirements within 72 hours, or if they are disqualified, an alternate winner may be selected from among the remaining eligible entries.

Any and all federal, state, and local taxes and other fees or costs associated with winning the Prize are the sole responsibility of the winner.

If required by law, Sponsor will issue an IRS Form 1099 to the winner for the value of the Prize.

By participating, you agree that Sponsor and its designees may use your name, likeness, and/or biographical information for advertising and promotional purposes in any media worldwide without limitation or additional compensation, unless prohibited by law.

Void Where Prohibited: This Promotion is void in any jurisdiction where prohibited by law.

‍No Liability: Sponsor is not responsible for technical malfunctions, lost or misrouted entries, or any other issues beyond Sponsor’s reasonable control.

‍Disqualification: Sponsor reserves the right to disqualify any individual it finds to be attempting to tamper with the entry process, game tokens, or otherwise violate these Official Rules.

‍Cancellation/Modification: Sponsor may, in its sole discretion, cancel, terminate, modify, or suspend the Promotion in the event the Promotion is compromised or cannot be completed as planned.

Any personal information you submit in connection with this Promotion is subject to Sponsor’s Privacy Policy, located at https://www.notablehealth.com/privacy-policy. By entering, you consent to Sponsor’s collection and use of your personal information for administration of this Promotion and for other purposes outlined in the Privacy Policy.

By participating, you agree to release and hold harmless Sponsor, its parent, subsidiaries, affiliates, advertising and promotion agencies, and all respective officers, directors, employees, representatives, and agents (collectively, “Released Parties”) from any and all liability for any claims, injuries, losses, or damages of any kind arising out of or related to participation in the Promotion or receipt, use, or misuse of any Prize.

Except where prohibited, you agree that any and all disputes arising out of or in connection with this Promotion shall be resolved individually, without resort to any form of class action, and exclusively by the appropriate state or federal courts located in San Mateo, California. All issues and questions concerning these Official Rules shall be governed by the laws of California, without giving effect to any conflict of law rules.
