1. Introduction

Welcome to Peak™, a program crafted to guide, support, and reward your Notable partnership journey.

At Notable, we believe that to drive success in your organization, great technology is not enough. It’s just one piece of the puzzle. Having the right implementation path is just as important. Success in leveraging healthcare technology is about people and process: proper planning, communication, change management, stakeholder alignment, strategic alignment, training, and measuring business outcomes.

Peak™ is about bringing Notable’s experiences to enhance your deployments, to enable speed to value, to accelerate impact, to engage you in the Notable community to learn and share best practices. We bring you our proven playbook for implementation, as well as create win-win incentives for success, where you accumulate incentives like contract discounts as well as things like participation in our tech talks, our customer advisory boards, the more that you are leveraging these best practices.

By participating in this program, you agree to the following terms and conditions (“Program Terms”). Please read them carefully to understand the mechanics and expectations of the program. These Program Terms outline the provisions, requirements, and benefits of customers’ participation in Peak™, ensuring a clear and beneficial partnership.

2. Eligibility

All Notable customers are eligible to participate in Peak. By participating in this program, you will not only gain access to tailored support and expertise, but also may have the opportunity to earn financial and other incentives related to the Notable products and services. Financial incentives may also be available under Peak if noted on a relevant Order Form.

3. Program Features

PeakTM is an innovative program designed by Notable to assist our valued customers in achieving optimal implementation and utilization of our platform. This program is uniquely structured to guide the customer towards best practices, ensuring the customer maximizes the benefits of our technology. As a valued PeakTM participant, you have an exclusive opportunity to achieve quick and seamless implementation of our products and services, along with ongoing benefits designed to maximize their impact. Our commitment to your success is reflected in the structured guidelines and goals of this program, and additional guidelines and goals may be provided to you separately in conjunction with a Statement of Work or Order Form. In recognition of your efforts to adhere to these guidelines and achieve these goals, we are pleased to offer a range of benefits and incentives. These may include both product enhancements and financial incentives, tailored to support and accelerate your journey with Notable. Non-financial incentives and support that may be made available under PeakTM based on your participation may include some or all of the following:

  1. Bonus Noteworthy invitations, including conference pass, 2-night hotel stay, and transportation to and from SFO airport
  2. Your Profile in the Notable Newsletter.
  3. Strategic consulting - digital transformation workshop led by the Notable Solutions Team
  4. Noteworthy To You - a NWTY session for non-executives at customer sites.
  5. Executive on-site at Notable’s headquarters to meet the engineers
  6. Noteworthy customer spotlight session
  7. Early access to beta-test new product features
  8. Public Relations Support
  9. "Founder's Hour" - Fireside chats with Notable co-founders Pranay, Adam, and/or Justin
  10. Priority invitation to events hosted by Notable

3.1 Features not tied to specific milestone completion, may include:

  1. Go-live support:
    1. Communications and change management templates to support stakeholder alignment (partnership overview deck, welcome document)
    2. Workflow documentation for all workflows powered by Notable solutions
    3. Collection of baseline metrics
    4. Training sessions (hosted, recorded training sessions)
    5. Education materials (posters, scripting, FAQ documents)
    6. Technical support
    7. Success Memo, a Notable document envisioning the future North Star for the partnership, success metrics, and outcomes
  2. Post-live support:
    1. Notable product reporting
    2. Product insights
    3. Benchmarking
    4. Quarterly Business Review preparation support

Available financial incentives will be set forth in a Statement of Work or Order Form between you and Notable. 

The incentives referenced in Section 3 above may be updated from time to time by Notable in its sole discretion, and such changes shall be effective as of the date this page is updated.

You must remain in compliance with these Program Terms and all requirements set forth herein to remain eligible for the incentives set forth above.

4. Customer Obligations

  1. Level 1 PeakTM Agility - Make your go-live quick and successful
    • Finalization of Project and Partnership Governance
    • Baseline metrics shared by Customer for benchmarking
    • Notable is granted necessary access  to Customer systems for each Flow to be implemented
    • On-site Pre-Live communication campaign
    • 1+ Certifications on Flow Studio for Reporting and Support per flow
    • 2 Flow Builder Training Registrations 
    • Successful Go live of flow
  2. Level 2 PeakTM Momentum - Sustain momentum and share successes
    • Stakeholder meetings held regularly
    • Celebrate and share progress internally at Customer site
    • Public celebration of your Go-live
    • Regular QBRs with stakeholders
    • Sign off on success metrics per flow at QBRs
    • Partnership Roadmap defined at QBRs: New potential flows identified
    • Support portal live on Zendesk
    • User engagement: 1+ users using Flow Studio for reporting and support, and to develop Forms and/or messages
    • 1 Notable AI Agent live
    • Share data for benchmarking with the Notable community
    • 2+ Flow Builder certified team members
  3. Level 3 PeakTM Experience - Learn and iterate
    • On-site shadowing to discover optimization opportunities
    • Publish your first internally built Flow
    • Adopt Notable AI Agents in 2 or more operational areas (eg InPatient, Ambulatory, PeriOp, Pharmacy)
    • Deploy 5+ Flows
    • Participate in 1 Case Study 
    • Participate in 2 reference calls annually
    • Annual Noteworthy presentation 
  4. Level 4 PeakTM Impact - Convert your wins into value
    • Participate in 2 Case Studies
    • 1 custom content created by the customer adopted by the Flow Builder community
    • Develop 3+ Custom Flows that are net new to Notable
    • Adopt Notable AI Agents in 3 or more operational areas (eg InPatient, Ambulatory, PeriOp, Pharmacy)
    • Participate in 4 reference calls annually
    • 2 Annual Presentations/Webinars Featuring Notable
5. Disclaimer

Customers acquire no vested right or entitlement to the continued availability of any particular reward, benefit or redemption level. Notable in its sole discretion can, restrict, suspend, amend, extend or otherwise alter these Program Terms at any time and without prior notice. If these Program Terms have been updated, Notable will provide the customer with the new Program Terms and note the date that they were last updated, and no change will be effective until such change has been posted for at least ten (10) days.

In the event that PeakTM is discontinued or terminated for any reason, Notable will notify customers’ designated project manager or other authorized agent specified in writing by the customer.

6. Other Terms and Conditions

These Program Terms are in addition to the underlying services agreement entered into between a customer and Notable (“Underlying Agreement”), and shall be read together with such Underlying Agreement as one and the same instrument; provided, that if there is a conflict between any of these Program Terms or the Underlying Agreement, the Underlying Agreement shall prevail. For the avoidance of doubt, in the event of any action, proceeding or other dispute arising from or relating to any Program Terms, such dispute shall be settled in accordance with the provisions and procedures set forth in the Underlying Agreement.
