This flow addresses a registry of patients with suspected open or overdue care gaps. It runs a clinical algorithm for one specific care gap to confirm the patient's true status in the EMR—determining if the patient is overdue or if a data point was missed, making them compliant or closed for that care gap. It then takes the next best action of either exporting the closure/exclusion evidence found to the EMR (e.g. evidence of a mammogram this year, or evidence of a bilateral mastectomy that excludes them from the breast cancer screening measure) or outreaching the patient to schedule an appointment to close gap.
